If you file a valid claim, you will be entitled to receive an award from the Settlement Fund. Payments will be based on:
- Whether you experienced a charging problem that, in good faith, you believe to be related to a defective Charging Port;
- How long you had the Covered Cell Phone when you first experienced the charging problem;
- Whether you contacted Motorola concerning the charging problem;
- If you have documentary proof of ownership of the Covered Cell Phone;
- The amount that you paid to replace your Covered Cell Phone, if you replaced the Covered Cell Phone with another cell phone as a result of the charging problem;
- The amount that you paid to repair your Covered Cell Phone, if you paid someone to repair your Covered Cell Phone as a result of the charging problem; and
- The amount that you paid for a replacement charger, if you purchased a new replacement charger for your Covered Cell Phone as a result of the charging problem.

Currently, you must submit the completed hard copy Claim Form and Affirmation, postmarked by March 11, 2010, to the Settlement Administrator at the address listed on the form. Please note that the claim deadline may be extended. If the deadline is extended, an update will be posted on this site.
When submitting a claim you must fully answer each of the questions that are applicable to you, and verify your claim under penalty of perjury. Failure to complete the relevant sections and verify your claim under penalty of perjury may result in the denial of your claim.
The Settlement Administrator reserves the right to request additional information regarding any submission.
